Kligman Solution · Ferrymead, Christchurch
Prescription Kligman Solution for melasma
Melasma can be stubborn. It can fade, then return with sun, hormones, heat or inflammation. Kligman Solution is a prescription topical treatment that targets several causes of excess pigment at the same time.
At Satini Cosmetic Clinic, your formula is prescribed and monitored by a doctor. Our customised version combines hydroquinone, tretinoin and low-concentration dexamethasone. We also add kojic acid to strengthen pigment control.
What improvement may look like
Clearer, more even-looking skin
Kligman Solution works gradually. You may notice dark patches soften and your overall tone look more even when you use the formula consistently and protect your skin every day.
What is Kligman Solution?
A four-part prescription approach to melasma
Kligman’s formula is a well-established prescription treatment for melasma. It combines ingredients that slow pigment production, improve skin turnover and reduce inflammation.
The traditional formula contains hydroquinone, tretinoin and dexamethasone. At Satini Cosmetic Clinic, we add kojic acid for extra pigment control. Your formula is compounded for you and supplied only after a doctor has assessed your skin.
Kligman Solution may help you with:
- Melasma
- Hormonal pigmentation
- Pregnancy-related pigment after pregnancy
- Recurring brown facial patches
- Post-inflammatory pigmentation
- Uneven dark spots
- Stubborn pigment after other skincare
- Maintenance after a professional pigment programme
What is inside your formula?
Four ingredients. Four complementary actions.
Each ingredient has a different role. Together, they target pigment more effectively than a single brightening ingredient alone.
Hydroquinone
The main pigment-reducing ingredient. It blocks tyrosinase, an enzyme your skin uses to produce melanin. This helps existing dark patches fade and slows new pigment formation.
Tretinoin
A prescription form of vitamin A. It speeds up skin-cell turnover and helps the other ingredients work more effectively. It can cause dryness or irritation, so your routine is introduced gradually.
Dexamethasone
A low-concentration corticosteroid used to calm inflammation and reduce irritation from the active ingredients. It must be used for the prescribed time because prolonged steroid use can thin or sensitise facial skin.
Kojic acid
At Satini, we add kojic acid to enhance pigment control. Kojic acid also reduces tyrosinase activity and can support a brighter, more even complexion.
Your treatment journey
How Kligman Solution works at Satini
This is not an over-the-counter fading cream. You receive a medical plan, clear instructions and follow-up.
Doctor consultation
We confirm that the pigment is melasma or another suitable condition. We review your skin type, triggers, medicines, pregnancy plans and current skincare.
Prescription
Your doctor prescribes a compounded formula with hydroquinone, tretinoin, dexamethasone and added kojic acid. The exact plan is chosen for your skin.
Slow introduction
You begin with small amounts and a lower frequency. We increase use only when your skin is comfortable. This helps reduce irritation.
Medical review
We assess your response, check for irritation and adjust the plan. Hydroquinone and steroid-containing formulas should not be used continuously without review.
Maintenance
Once your pigment improves, we may reduce the formula, use it intermittently or change you to non-hydroquinone maintenance products and daily SPF.
Combination treatment
Kligman Solution and tranexamic acid
Melasma has several drivers. For some people, topical treatment alone is not enough. Your doctor may combine Kligman Solution with tranexamic acid as a topical product or a tablet.
Tranexamic acid works through a different pathway. It reduces signals linked with pigment production and the vascular component of melasma. The combination may be useful when your melasma is resistant, recurrent or strongly influenced by hormones.
Your doctor may consider tranexamic acid when:
- Your melasma keeps returning
- Your pigment has a strong hormonal pattern
- Topical treatment has only partly helped
- You have widespread facial melasma
- You can safely use oral or topical tranexamic acid
- You are committed to daily light protection
What to expect
Results, irritation and recovery
Early adjustment
You may notice dryness, mild peeling, redness or stinging when you begin. Starting slowly and using the right moisturiser can make the treatment easier to tolerate.
Gradual improvement
You may begin to see a brighter, more even tone over several weeks. Stubborn or long-standing melasma often needs a longer, carefully monitored plan.
Long-term control
Melasma is a chronic condition. Your result is easier to maintain when you use SPF 50+, protect against visible light and follow your maintenance plan.
Compare your options
Kligman Solution, Cosmelan, Dermamelan or IPL?

IPL is generally not our first choice for melasma because heat and inflammation can trigger recurrence in some skin types.

Frequently asked questions
Questions about Kligman Solution
What is Kligman Solution?
It is a prescription topical treatment for melasma. The traditional formula combines hydroquinone, tretinoin and dexamethasone. At Satini, we also add kojic acid to strengthen pigment control.
Why is Kligman Solution called the Gold Standard for melasma?
It treats pigment through several pathways at once. Hydroquinone reduces melanin production, tretinoin improves skin turnover and the corticosteroid reduces inflammation. Medical reviews continue to describe this triple combination as the leading topical treatment for melasma.
Is Kligman Solution prescription only?
Yes. It contains prescription-strength hydroquinone, tretinoin and a corticosteroid. A doctor must confirm that it is suitable, prescribe it and review how your skin responds.
How do I use it?
Your doctor gives you a personalised schedule. You usually begin with a very small amount and use it less often at first. The frequency can increase if your skin tolerates it. Do not copy another person’s routine.
How quickly will I see results?
You may notice early improvement over several weeks. More stubborn melasma can take longer. Your result depends on pigment depth, triggers, skin type, consistency and light protection.
Can I use Kligman Solution long term?
Not continuously without medical review. Hydroquinone and corticosteroids need planned breaks or intermittent use. Once your pigment improves, your doctor may change you to a safer long-term maintenance routine.
Can I combine it with tranexamic acid?
Sometimes. Your doctor may add topical or oral tranexamic acid when melasma is resistant or recurrent. Oral tranexamic acid needs a careful medical and clotting-risk assessment and is not suitable for everyone.
Can I use Kligman Solution during pregnancy?
No. Tretinoin and hydroquinone are generally avoided during pregnancy, and your treatment must be reviewed if you are pregnant, breastfeeding or planning a pregnancy.
Will my melasma come back?
It can. Melasma is often triggered by UV light, visible light, heat and hormones. Daily SPF 50+, hats, trigger control and maintenance skincare help you protect your improvement.
Is Kligman Solution better than Cosmelan or Dermamelan?
It is different rather than universally better. Kligman is a prescription home treatment. Cosmelan and Dermamelan combine an in-clinic mask with a structured home programme. Your doctor will recommend the option that best suits your pigment, skin sensitivity and lifestyle.
What side effects should I expect?
Dryness, peeling, redness and mild stinging are common when you start. Strong irritation, swelling, blistering, marked darkening or skin thinning need medical review.
